I did the survey, but for whatever reason it almost looked like it didn't take, and I didn't see a link to this mini kit... Do you have to be logged in to do it? I wasn't logged in, so if that's the case maybe I'll need to do it again?
Just took the survey (I was logged in) and when I hit the Submit button the next page looked blank, I had to scroll up to the top of the page as that is where the link was. The page had loaded in the middle not at the top.

Hi Lizanne. If you visit the survey page here, it should tell you if you already took it (and if you want to edit your answers). If not, your answers probably didn't go through.
