How to make plastic slip-in pockets in Photoshop

Does anyone know where there is a tutorial for this or how to do it? I want some of these but not in the shapes that I have been able to find so I'm wanting to just make my own but don't have the slightest clue where to start.

Karry, I know it has been a while since you asked this question but discovered this link. I have not tried but it may help if you have not already discovered how to make the pockets.
